Disease-Specific Discharge Follow-Up Calls

Our disease-specific post-discharge patient calls are designed to target patients with a specific disease or high-risk category; such as asthma, diabetes, COPD, CHF, renal disease, hypertension and stroke.  Our nursing team follows these patients for a 30-day period, acting as a liaison between the patient and their providers, ensuring a safe and healthy transition from hospital to home and help reduce unnecessary readmissions.

High-Volume Discharge Follow-Up Calls

Within 48-72 hours post-discharge our nursing team will call your patient and conduct a follow-up evaluation based on questions designed to report 1) the patient's current condition; 2) the patient's perception of pain management; the patient's perception of the care they received; 3) the patient's understanding of their discharge instructions; and 4) if the patient has scheduled a follow-up appointment with their PMD/Specialist.
